Obviously everyone knows there are costs. It’s free (at the point of use) but almost noone needs the brackets to be explicit. The point being that it’s more efficient and you continue to get it even if you have no “gross income” because we’re not monsters.
Also how many countries have a “fixed percentage” with literally no allowances or brackets. I can’t think of any.

No country has “free” healthcare (afaik), people just have a fixed percentage amount deducted from their gross income every month.

Well, yeah, but if you are unemployed you still get to benefit from it for free in Spain, so it’s a collectively paid free healthcare.
Kids also get to benefit from it without depending on their parents’ jobs or anything.
We pay the infrastructure that we can provide free healthcare. It’s free in the receiving end.
