You give a general prescription and see if it fixes the eye crossing or drift. If not, up the prescription.
They can also give kids eye drops that relax the eyes and then they use a machine to look at their eyes and get a more precise reading and prescription.
There’s instruments that use wavefront aberration to automatically measure your eyes for corrective lenses. As an adult, I’ve had eye doctors use one to get an initial guess to speed up the manual testing process. It’s not quite as accurate, but it’s close enough to get a decent prescription for an infant.
Yeah they had one that was an attachment to a smartphone at my kid’s pediatrician. The assistant just snaps a picture of her eye and it can measure the refractive index from its special camera and it gave a reading right in the app almost instantly
